Physical Therapy
This service provides assistance to individuals with physical limitations to acquire, maintain
Community Choice
The community choice service component is the provision of assistance and support necessary for an individuals
Nursing (RN/LVN)
Our nursing services are available to individuals enrolled in the HCS/TxHmL program
Adaptive Aids
This service provides individuals who are enrolled in the HCS/TxHmL program with devices, controls, or appliances
