• Psychiatric Evaluation & Medication Management: A structured process for psychiatric evaluation, medication management, and coordinated collaboration among psychiatric and behavioral health professionals.
• Clinical Assessment & Treatment Planning: Clinical interviews, diagnostic assessments by authorized professionals, person-centered treatment planning, risk assessment, and ongoing evaluation of progress toward treatment goals.
• Family Support & Care Coordination: Clear and timely communication, appropriate collaboration with authorized family members, and coordinated care among the individuals, providers, and systems involved in supporting the individual’s needs.
• Mental Health Targeted Case Management: Coordination that assists eligible individuals in accessing and connecting with medical, social, behavioral, educational, and community-based services identified in their plan of care.
• Mental Health Rehabilitative Services: Skills training and rehabilitative support designed to enhance functional abilities, promote independence, improve symptom management, and support progress toward person-centered goals.
• Outpatient Psychotherapy: Evidence-informed individual and family therapy provided by independently licensed LPCs, LCSWs, and LMFTs within their scope of competence and authorized clinical privileges.
